The Popfit application that you are about to use is a clinical improvement initiative of the Nepean Family Obesity
Service (NFOS), developed by Vulsen and is supported by the University of Sydney and the Nepean Blue Mountains
Local Health District. It is a family oriented mobile application aimed towards providing incentives for
an entire family to live a better and healthier lifestyle.
Using this application is entirely voluntary and it will allow for an easier and more convenient method
for you to keep track of and monitor your daily and weekly health and fitness goals set by your physician,
dietitian, fitness trainer, as well as other health professionals from the NFOS. Popfit further provides
a social platform for the family to send encouragements as well as to motivate each other through the exchange
of communications and the sharing of personal health and fitness data between family members. This application
also serves as a communication bridge between you as a user to your NFOS health team. You may send and receive
private messages; asking for information, advice or feedback from the various health professionals at the
NFOS.
The data that you enter into Popfit belongs to you and you may choose to share the data electronically in
real-time with your healthcare team. Sharing your data electronically in real time will allow for feedback
and adjustments to be made to your daily and weekly goals by your NFOS health team. Data you share on the
social platform can further be made accessible to your NFOS health team, which can allow them to send feedback
and provide encouragements to you and your family.
Should you decide to share your data electronically in real-time with your healthcare team, the data will
be uploaded and stored in a secure cloud based server and will be available to your health care team to access
at any time. However, your healthcare team will not be responsible to act and make clinical decisions based
on the data they may have access to, unless you request them to do so, each time.
This agreement is binding between you and the Nepean Family Obesity Service of Nepean Blue Mountains Local
Health Districts. By allowing sharing of your data electronically with your healthcare team, you give permission
to the staff working in the NFOS to use this data for your clinical care, as well as for clinical audits,
clinical research and administrative purposes. This would be binding indefinitely, or until you revoke this
agreement.
